Letter of Commitment for Work Resumption of Workers Returning (Coming) to

Beijing

Leading Group of Chaoyang District on COVID-19 Epidemic Prevention and

Control, Work and Production Resumption:

I, having consummated a 14-day (from ____ to ___) home quarantine, with an

obrvation of normal body temperature and free of other suspected symptom and

without living with any person staying in Beijing for less than 14 days after returning or

coming from out of town or staying in China for less than 14 days after coming from or

visiting any most verely epidemic-affected areas of the departed country, after my

returning (coming) to Beijing from ____ City (District/County), ___ Province

(Municipality/Autonomous Region), ___ (Country/Region) on ___ (MM) ___ (DD), hereby

apply for work resumption with the solemn commitments as follows:

1. Conscientiously proceed with taking personal safety precautions. Voluntarily put

into practice the “One-Meter” Initiative. Wear facemasks, stay at least One Meter

away from others and avoid talking or dining with others as strict precautionary

measures in the places such as public transit means, elevator, office and dining hall. For

any travel out of Beijing as necessitated after the work resumption, voluntarily report it

to the employer in advance and voluntarily submit the travel itinerary to the residential

neighborhood authority and the employer ASAP and then start a 14-day home

quarantine after returning to Beijing. For any act of living with any person returning to

Beijing recently or coming to China recently from or after any visit of the most verely

epidemic-affected areas of the departed country, report it to the employer in time and

voluntarily start a 14-day home quarantine for obrvation. For any abnormal physical

symptom, ek medical rvices in time and report it to the governing authority.

2. Be highly cooperative in the epidemic prevention and control efforts launched

by the residential neighborhood authority, the employer and the office building

property rvice company. In tandem with the related personnel, conscientiously fill in

the related information in a timely and accurate manner, voluntarily accept body

temperature detections and make registrations when entering and exiting residential

and office buildings and earnestly proceed with various screening and inspection

efforts on epidemic prevention and control.

3. Resolutely resist rumors. Refrain from believing or spreading rumors, forwarding

any fal epidemic-related information as posted at Weibo, WeChat or other online We

media platforms, fabricating or posting any fal information or intently inflaming

panic. Unwaveringly resolute any COVID-19-related information without official

substantiation and make concerted efforts to maintain the public order.

4. Voluntarily fulfill social responsibilities. Never conceal or misreport any personal

or family health status, travel itinerary out of Beijing or other information on epidemic

prevention and control. Make various efforts to facilitate epidemic prevention and

control while taking earnest lf-precautions.
