第一章總則
第一條為規范藥品生產質量管理，根據《中華
人民共和國藥品管理法》和《中華人民共和國藥品
管理法實施條例》的規定，制定本規范。
第二條企業應建立藥品質量管理體系。該體系
包括影響藥品質量的所有因素，是確保藥品質量符
合預定用途所需的有組織、有計劃的全部活動總和。
第三條本規范作為質量管理體系的一部分，是
藥品生產管理和質量控制的基本要求，以確保持續
穩定地生產出適用于預定用途、符合注冊批準或規
定要求和質量標準的藥品，并最大限度減少藥品生
產過程中污染、交叉污染以及混淆、差錯的風險。
第四條本規范為藥品生產質量管理的基本要求。
附錄為藥品生產質量管理的特殊要求，適用于相關
的藥品或生產質量管理活動，可根據情況適時修訂。
第五條本規范不包括有關環境保護、勞動安全
等管理要求。
第六條企業應誠實守信地遵守本規范。
第七條企業可以采用經過驗證的替代方法，達
到本規范的要求。
第二章質量管理
第一節原則
第八條企業應建立并實施符合質量管理體系要
求的質量目標，將藥品注冊中有關安全、有效和質
量可控的所有要求，系統地貫徹到藥品生產、控制
及產品放行、發運的全過程中，確保所生產的藥品
適用于預定的用途，符合注冊批準或規定要求和質
量標準。
第九條企業高層管理人員應確保實現既定的質
量目標，各部門不同層次的人員以及供應商、經銷
商應共同參與并承擔各自的責任。
第十條企業應配備足夠的、符合要求的人員、
廠房、設施和設備，為實現質量目標提供必要的條
件。
第二節質量保證
第十一條質量保證是質量管理體系的一部分。
企業必須建立質量保證系統，應以完整的文件形式
明確規定，并監控其有效性。
第十二條質量保證應確保符合下列要求：
1.藥品的設計與研發應考慮本規范的要求；
2.明確規定生產管理和質量控制活動，保證本
規范的實施；
3.明確管理職責；
4.保證生產以及采購和使用的原輔料和包裝材
料正確無誤；
5.確保中間產品所需的控制以及其它中間控制
得到實施；
6.確保驗證的實施；
7.嚴格按各種書面規程進行生產、檢查、檢驗
和復核；
8.只有經質量受權人批準，每批產品符合注冊
批準以及藥品生產、控制和放行的其它法規要求后，
方可發運銷售。產品放行審核包括對相關生產文件
和記錄的檢查以及對偏差的評估；
9.有適當的措施保證貯存、發運和隨后的各種
處理過程中，藥品質量在有效期內不受影響；
10.制定自檢操作規程，定期檢查評估質量保
證系統的有效性和適用性。
第十三條藥品生產質量管理的基本要求：
1.明確規定生產工藝，系統地回顧并證明其可
持續穩定地生產出符合標準的產品；
2.關鍵生產工藝及其重大變更均經過驗證；
3.已配備所需的資源，至少包括：
(1)具有適當的資質并經培訓合格的人員；
(2)足夠的廠房和空間；
(3)適用的設備和維修保障；
(4)正確的原輔料、包裝材料和標簽；
(5)批準的工藝規程和操作規程；
(6)適當的貯運條件。
4.使用清晰準確的文字，制定相關設施的操作
規程；
5.操作人員經過培訓，能按操作規程正確操作；

和操作規程所要求的所有步驟均已完成，產品數量
和質量符合預期要求，重大偏差經過調查并有完整
記錄；
7.能夠追溯批產品歷史的完整批記錄和發運記
錄，應妥善保存、便于查閱；
8.盡可能降低藥品發運的質量風險；
9.建立藥品召回系統，可召回任何一批已發運
銷售的產品；
10.審查藥品的投訴，調查導致質量缺陷的原因，
并采取措施，防止再次發生類似的質量缺陷。
第三節質量控制
第十四條質量控制包括相應的組織機構、文件
系統以及取樣、檢驗等，確保物料或產品在放行前
完成必要及相關的檢驗，確認其質量符合要求后，
方可使用或發運。
第十五條質量控制的基本要求：
1.應配備適當的設施、儀器、設備和經過培訓
的人員，有效、可靠地完成所有質量控制的相關活
動；
2.應有批準的操作規程，用于原輔料、包裝材
料、中間產品、待包裝產品
和成品的取樣、檢查、檢驗以及產品的穩定性考察，
必要時進行環境監測，以符合本規范的要求；
3.由經授權的人員按規定的方法對原輔料、包
裝材料、中間產品、待包裝產品和成品取樣；
4.檢驗方法應經過驗證或確認；
5.應有儀器或手工記錄，表明所需的取樣、檢
查、檢驗均已完成，偏差應有完整的記錄并經過調
查；
6.物料、中間產品、待包裝產品和成品必須按
照質量標準進行檢查和檢驗，并有記錄；
7.物料和最終包裝的成品應有足夠的留樣，以
備必要的檢查或檢驗；除最終包裝容器過大的成品
外，成品的留樣包裝應與最終包裝相同。
第四節質量風險管理
第十六條質量風險管理是對整個產品生命周期
進行質量風險的識別、評估、控制、溝通、回顧的
系統過程，運用時可采用前瞻或回顧的方式。
第十七條應根據科學知識及經驗對質量風險
進行評估，并將質量風險與保護患者的最終目標相
關聯，以保證產品質量。
第十八條質量風險管理應與存在風險的級別
相適應，確定相應的方法、措施、形式和文件。
第三章機構與人員
第一節原則
第十九條企業應建立管理機構，并有組織機
構圖。
質量管理部門應獨立于其它部門，履行質量保
證和質量控制的職責。根據企業的實際情況，質量
管理部門可以分別設立質量保證部門和質量控制部
門。
第二十條質量管理部門應參與所有與質量有
關的活動和事務，負責審核所有與本規范有關的文
件。質量管理部門人員的職責不得委托給其它部門
的人員。
第二十一條企業應配備足夠數量并具有適當
資質（含學歷、培訓和實踐經驗）的人員從事管理
和各項操作，應明確規定每個部門和每個崗位的職
責。所有人員應明確并理解自己的職責，熟悉與其
職責相關的要求，并接受必要的培訓，包括上崗前
培訓和繼續培訓。
第二十二條不同崗位的人員均應有詳細的書
面工作職責，并有相應的職權，其職能可委托給具
有相當資質的指定代理人。每個人所承擔的職責不
應過多，以免導致質量風險。崗位的職責不得有空
缺，重疊的職責應有明確的解釋。
第二節關鍵人員
第二十三條關鍵人員至少應包括企業負責人、
生產管理負責人、質量管理負責人和質量受權人。
關鍵人員應為企業的全職人員。
質量管理負責人和生產管理負責人不得互相兼
任。質量管理負責人和質量受權人可以兼任。
第二十四條企業負責人
企業負責人是藥品質量的主要責任人。為確保
企業實現質量目標并按照
本規范要求生產藥品，企業負責人應負責提供必要
的資源配置，合理計劃、組織
和協調，不得干擾和妨礙質量管理部門獨立履行其
職責。
第二十五條生產管理負責人
1.資質
生產管理負責人應至少具有藥學或相關專業本
科學歷（或中級專業技術職稱或執業藥師資格），
具有至少三年從事藥品生產的實踐經驗和至少一年
的藥品生產管理工作經驗，接受過與所生產產品相
關的專業知識培訓。
2.生產管理負責人應履行的主要職責：
(1)確保藥品按工藝規程和操作規程生產、貯存，
以保證藥品質量；
(2)確保嚴格執行工藝規程和生產操作相關的各
種操作規程；
(3)確保批生產記錄和批包裝記錄經過指定人員
審核并送交質量管理部門；
(4)確保廠房和設備的維護保養，以保持其良好
的運行狀態；
(5)確保完成各種必要的驗證工作；
(6)確保生產相關人員經過必要的上崗前培訓和
繼續培訓，并根據實際需要調整培訓內容。
第二十六條質量管理負責人
1.資質
質量管理負責人應至少具有藥學或相關專業本
科學歷（或中級專業技術職稱或執業藥師資格），
具有至少五年的藥品生產質量管理實踐經驗和至少
一年的藥品質量管理工作經驗，接受過與所生產產
品相關的專業知識培訓。
2.質量管理負責人應履行的主要職責：
(1)確保原輔料、包裝材料、中間產品、待包裝
產品和成品符合注冊批準的要求和質量標準；
(2)確保完成和監督批記錄的放行審核；
(3)確保完成所有必要的檢驗；
(4)批準質量標準、取樣方法、檢驗方法和其它
質量管理規程；
(5)審核和批準所有與質量有關的變更；
(6)確保所有重大偏差和檢驗結果超標已經過
調查并得到及時處理；
(7)批準并監督委托檢驗；
(8)監督廠房和設備的維護情況，以保持其良
好的運行狀態；
(9)確保完成各種必要的驗證工作，審核和批
準驗證方案和報告；
(10)確保完成自檢；
(11)批準和評估物料的供應商；
(12)確保所有與產品質量有關的投訴已經過調查，
并得到及時正確的處理；
(13)確保完成產品的持續穩定性考察計劃，提
供穩定性考察的數據；
(14)確保完成產品質量回顧分析；
(15)確保質量控制和質量保證人員都已經過必
要的上崗前培訓和繼續培訓，并根據實際需要調整
培訓內容。
第二十七條生產管理負責人和質量管理負責
人通常有下列共同的質量職責：
1.審核和批準操作規程和文件；
2.審核和批準產品的工藝規程；
3.監督廠區衛生狀況；
4.確保關鍵設備經過確認、儀表校準在有效期
內；
5.確保完成生產工藝驗證；
6.確保企業所有相關人員都已經過必要的上崗
前培訓和繼續培訓，并根據實際需要調整培訓內容；
7.批準并監督委托生產；
8.確定和監控物料和產品的貯存條件；
9.保存記錄；
10.監督本規范執行狀況；
11.為監控某些影響產品質量的因素而進行檢
查、調查和取樣。
第二十八條質量受權人
1.資質
質量受權人應至少具有藥學或相關專業大學本
科的學歷（或中級專業技術職稱或執業藥師資格），

2.質量受權人應具有必要的專業理論知識，并
經過與產品放行有關的培訓，方能獨立履行其職責。
3.主要職責：
(1)必須保證每批已放行產品的生產、檢驗均符
合相關法規、藥品注冊批準或規定的要求和質量標
準；
(2)在任何情況下，質量受權人必須在產品放行
前對上述第(1)款的要求作出書面承諾，并納入批記
錄。
應制定操作規程確保質量受權人的獨立性，企業
負責人和其他人員不得干擾質量受權人獨立履行職
責。
第三節培訓
第二十九條企業應指定部門或專人負責培訓管
理的工作，應有經生產管理負責人或質量管理負責
人審核或批準的培訓方案或計劃，培訓記錄應予保
存。
第三十條與藥品生產、質量有關的所有人員都
應經過培訓，培訓的內容應與每個崗位的要求相適
應。除進行本規范理論和實踐的基礎培訓外，還應
有相關法規、相應崗位的職責、技能培訓和繼續培
訓，繼續培訓的實際效果應定期評估。
第三十一條高風險操作區（如：高活性、高毒
性、傳染性、高致敏性物料的生產區）的工作人員
應接受專門的培訓。
第四節人員衛生
第三十二條所有人員都應接受衛生要求的培訓，
企業應建立人員衛生操作規程，最大限度地降低人
員對藥品生產造成的污染風險。
第三十三條為滿足企業的各種需要，人員衛生
操作規程應包括與健康、衛生習慣及人員著裝相關
的內容。生產區和質量控制區的工作人員應正確理
解相關的衛生操作規程。企業應采取措施確保人員
衛生操作規程的執行。
第三十四條企業應采取措施保持人員良好的健
康狀況，并有健康檔案。所有人員在招聘時均應接
受體檢。初次體檢后，應根據工作需要及人員健康
狀況安排體檢。直接接觸藥品的生產人員應每年至
少體檢一次。
第三十五條企業應采取適當措施，避免傳染
病或體表有傷口的人員從事直接接觸藥品或對藥品
質量有不利影響的生產。
第三十六條應限制參觀人員和未經培訓的人
員進入生產區和質量控制區；不可避免時，應事先
就個人衛生、更衣等要求進行指導。
第三十七條任何進入生產區的人員均應按規
定更衣。工作服的選材、式樣及穿戴方式應與所從
事的工作和空氣潔凈度級別要求相適應。
第三十八條進入潔凈生產區的人員不得化妝
和佩帶飾物。
第三十九條生產區、倉儲區應禁止吸煙和飲
食，禁止存放食品、飲料、香煙和個人用藥品等非
生產用物品。
第四十條操作人員應避免裸手直接接觸藥品、
與藥品直接接觸的包裝材料和設備的表面。
第四章廠房與設施
第一節原則
第四十一條廠房的選址、設計、布局、建造、
改造和維護必須符合藥品生產要求，應能最大限度
避免污染、交叉污染、混淆和差錯，便于清潔、操
作和維護。
第四十二條應根據廠房及生產防護措施綜合
考慮選址，廠房所處的環境應能最大限度降低物料
或藥品遭受污染的風險。
第四十三條企業應有整潔的生產環境；廠區
的地面、路面及運輸等不應對藥品的生產造成污染；
生產、行政、生活和輔助區的總體布局應合理，不
得互相妨礙；廠區和廠房內的人、物流走向應合理。
第四十四條應對廠房進行適當維護，應確保
維修活動不影響藥品的質量。應按照詳細的書面操
作規程對廠房進行清潔或必要的消毒。
第四十五條廠房應有適當的照明、溫濕度和
通風，確保生產和貯存的藥品質量以及相關設備性
能不會直接或間接地受到影響。
第四十六條廠房的設計和安裝的設施應能有
效防止昆蟲或其它動物進入。應采取必要的措施，
避免所使用的滅鼠藥、殺蟲劑、煙熏劑等對設備、
物料、中間產品、待包裝產品或成品造成污染。
第四十七條應采取適當措施，防止未經批準人
員的進入。生產、貯存和質量控制區不應作為非本
區工作人員的通道。
第四十八條應保存廠房、公用設施、固定管道
建造或改造后的竣工圖紙。
第二節生產區
第四十九條為降低污染和交叉污染，廠房、生
產設施和設備應根據所生產藥品的特性、工藝流程
及相應潔凈度級別要求合理設計、布局和使用，并
符合下列要求：
1.應綜合考慮藥品的特性、工藝和預定用途等
因素，確定廠房、生產設施和設備數個產品共用的
可行性，并有相應評估報告；
2.生產特殊性質的藥品，如高致敏性藥品（如
青霉素類）或生物制品（如卡介苗或其它用活性微
生物制備而成的藥品）必須采用專用和獨立的廠房、
生產設施和設備。青霉素類藥品產塵量大的操作區
域應保持相對負壓，排至室外的廢氣應經凈化處理
并符合要求，排風口應遠離其它空氣凈化系統的進
風口；
3.生產β-內酰胺結構類、性激素類避孕藥品必
須使用專用設施（如獨立的空氣凈化系統）和設備，
并與其它藥品生產區嚴格分開；
4.生產某些激素類、細胞毒性類、高活性化學
藥品應使用專用設施（如獨立的空氣凈化系統）和
設備；特殊情況下，如采取特別防護措施并經過必
要的驗證，上述藥品制劑則可通過階段性生產方式
共用同一生產設施和設備；
5.用于上述第2、3、4款中藥品生產的空氣凈化
系統，其排風應經凈化處理；
6.藥品生產廠房不得用于生產對藥品質量有不
利影響的非藥用產品。
第五十條生產區和貯存區應有足夠的空間，以
有序地存放設備、物料、中間產品、待包裝產品和
成品，避免不同藥品或物料的混淆，避免交叉污染，
避免生產或質量控制操作發生遺漏或差錯。
第五十一條應根據藥品品種、生產操作要求及
外部環境狀況配置空調凈化系統，使生產區有效通
風，并有溫度控制、必要的濕度控制和空氣凈化過
濾，保證藥品的生產環境。
潔凈區與非潔凈區之間、不同級別潔凈區之間
的壓差應不低于10帕斯卡，相同潔凈度級別不同功
能的操作間之間應保持適當的壓差梯度，以防止污
染和交叉污染。
口服液體和固體制劑、腔道用藥（含直腸用藥）
、表皮外用藥品生產的暴露工序區域及其直接接觸
藥品的包裝材料最終處理的暴露工序區域，應參照
“無菌藥品”附錄中D級潔凈區的要求設置，企業可
根據產品的標準和特性對該區域采取適當的微生物
監控措施。
第五十二條潔凈區的內表面（墻壁、地面、
天棚）應平整光滑、無裂縫、接口嚴密、無顆粒物
脫落，避免積灰，便于有效清潔和必要時進行消毒。
第五十三條各種管道、照明設施、風口和其
它公用設施的設計和安裝應避免出現不易清潔的部
位，應盡可能在生產區外部對其進行維護。
第五十四條排水設施應大小適宜，安裝防止
倒灌的裝置。應盡可能避免明溝排水；不可避免時，
明溝宜淺，以方便清潔和消毒。
第五十五條制劑的原輔料稱量通常應在專門
設計的稱量室內進行。
第五十六條產塵操作間（如干燥物料或產品
的取樣、稱量、混合、包裝等操作間）應保持相對
負壓，應采取專門的措施防止粉塵擴散、避免交叉
污染并便于清潔。
第五十七條用于藥品包裝的廠房或區域應合
理設計和布局，以避免混淆或交叉污染。如同一區
域內有數條包裝線，應有隔離措施。
第五十八條生產區應有適度的照明，目視操
作區域的照明應滿足操作要求。
第五十九條生產區內可設中間控制區域，但
中間控制操作不得給藥品帶來質量風險。
第三節倉儲區
第六十條倉儲區應有足夠的空間，以有序存
放待驗、合格、不合格、退貨或召回的原輔料、包
裝材料、中間產品、待包裝產品和成品等各類物料
和產品。
第六十一條倉儲區的設計和建造應確保良好

第六十四條如采用單獨的隔離區域貯存待驗物
料，待驗區應有醒目的標識，且只限于經批準的人
員出入。
不合格、退貨或召回的物料或產品應隔離存放。
如果采用其它方法替代物理隔離，則該方法應
具有同等的安全性。
第六十五條通常應有單獨的物料取樣區。取樣
區的空氣潔凈度級別應與生產要求一致。如在其它
區域或采用其它方式取樣，應能防止污染或交叉污
染。
第四節質量控制區
第六十六條質量控制實驗室、中藥標本室通常
應與生產區分開，生物檢定、
微生物和放射性同位素的實驗室還應彼此分開。
第六十七條實驗室的設計應確保其適用于預定
的用途，并能夠避免混淆和交叉污染，應有足夠的
區域用于樣品處置、留樣和穩定性考察樣品的存放
以及記錄的保存。
第六十八條必要時，應設置專門的儀器室，使
靈敏度高的儀器免受靜電、震動、潮濕等或其它外
界因素的干擾。
第六十九條處理生物或放射性樣品等特殊物品
的實驗室應符合國家的有關要求。
第七十條實驗動物房應與其它區域嚴格分開，
其設計、建造應符合國家有關規定，并設有獨立的
空氣處理設施以及動物的專用通道。
第五節輔助區
第七十一條休息室的設置不應對生產區、倉
儲區和質量控制區造成不良影響。
第七十二條更衣室和盥洗室應方便人員出入，
并與使用人數相適應。盥洗室不得與生產區和倉儲
區直接相通。
第七十三條維修間應盡可能遠離生產區。存
放在潔凈區內的維修用備件和工具，應放置在專門
的房間或工具柜中。
第五章設備
第一節原則
第七十四條設備的設計、選型、安裝、改造
和維護必須符合預定用途，應盡可能降低發生污染、
交叉污染、混淆和差錯，便于操作、清潔、維護，
以及必要時進行的消毒或滅菌。
第七十五條應建立設備使用、清潔、維護和
維修的操作規程，并保存相應的操作記錄。
第七十六條應保存設備采購、安裝、確認、
維修和維護、使用、清潔的文件和記錄。
第二節設計和安裝
第七十七條生產設備不得對藥品有任何危害，
與藥品直接接觸的生產設備表面應光潔、平整、易
清洗或消毒、耐腐蝕，不得與藥品發生化學反應或
吸附藥品，或向藥品中釋放物質而影響產品質量并
造成危害。
第七十八條藥品的生產和控制應配備具有適
當量程和精度的衡器、量具、儀器和儀表。
第七十九條應選擇適當的清洗、清潔設備，
以避免這類設備成為污染源。
第八十條設備所用的潤滑劑、冷卻劑等不得
對藥品或容器造成污染，應盡可能使用食用級或與
產品級別相當的潤滑劑。
第八十一條生產用模具的采購、驗收、保管、
維護、發放及報廢應制定相
應操作規程，設專人專柜保管，并有相應記錄。
第三節維護和維修
第八十二條設備的維護和維修不得影響產品
質量。
第八十三條應制定設備的預防性維護計劃和操
作規程，設備的維護和維修應有相應的記錄。
第八十四條經改造或重大維修的設備應進行重
新確認或驗證，符合要求后方可用于生產。
第四節使用、清潔和狀態標識
第八十五條主要生產和檢驗設備都應有明確的
操作規程。
第八十六條生產設備應在確認的參數范圍內使
用。
第八十七條應按詳細規定的操作規程清潔生產
設備。
生產設備清潔的操作規程應規定具體而完整的
清潔方法、清潔用設備或工具、清潔劑的名稱和配
制方法、去除前一批次標識的方法、保護已清潔設
備在使用前免受污染的方法、已清潔設備最長的保
存時限、使用前檢查設備清潔狀況的方法，使操作
者能以可重現的、有效的方式對各類設備進行清潔。
如需拆裝設備，還應規定設備拆裝的順序和方
法；如需對設備消毒或滅菌，還應規定消毒或滅菌
的具體方法、消毒劑的名稱和配制方法。還應規定
設備生產結束至清潔前所允許的最長間隔時限。
第八十八條已清潔的生產設備通常應在清潔、
干燥的條件下存放。
第八十九條用于藥品生產或檢驗用的主要或關
鍵設備，應有使用日志，記錄內容包括使用、清潔、
維護和維修情況以及日期、時間、所生產及檢驗的
藥品名稱、規格和批號等。
第九十條生產設備應有明顯的狀態標識，標明
設備編號和內容物（如名稱、規格、批號）；沒有
內容物的應標明清潔狀態。
第九十一條應盡可能將閑置不用的設備搬出生
產和質量控制區，有故障的設備應有醒目的狀態標
識。

第五節校準
第九十三條應確保生產和檢驗使用的關鍵衡器、
量具、儀表、記錄和控制設備以及儀器經過校準，
所得出的數據準確可靠。
第九十四條應按照操作規程和校準計劃定期
對生產和檢驗用衡器、量具、儀表、記錄和控制設
備以及儀器進行校準和檢查，并保存相關記錄。應
特別注意校準的量程范圍涵蓋實際生產和檢驗的使
用范圍。
第九十五條應使用標準計量器具進行校準，
且所用標準計量器具有可以溯源到國際或國家標準
器具的計量合格證明。校準記錄應標明所用標準計
量器具的名稱、編號、校準有效期和計量合格證明
編號，確保記錄的可追溯性。
第九十六條衡器、量具、儀表、記錄和控制
的設備以及儀器應有明顯的標識，標明其校準有效
期。
第九十七條超出校準合格標準的衡器、量具、
儀表、記錄和控制的設備以及儀器不得使用。
第九十八條如在生產、包裝、倉儲過程中使
用自動或電子設備的，應按操作規程定期進行校準
和檢查，確保其操作功能正常。校準和檢查應有相
應的記錄。
第六節制藥用水
第九十九條藥品生產用水應適合其用途，應
至少采用飲用水作為制藥用水。各類藥品生產選用
的制藥用水應符合《中華人民共和國藥典》的相關
要求。
第一百條飲用水應符合國家有關的質量標準，
純化水、注射用水應符合《中華人民共和國藥典》
的質量標準。
第一百零一條水處理設備及其輸送系統的設
計、安裝和維護應能確保制藥用水達到設定的質量
標準。水處理設備的運行不得超出其設計能力。
第一百零二條純化水、注射用水儲罐和輸送
管道所用材料應無毒、耐腐蝕；儲罐的通氣口應安
裝不脫落纖維的疏水性除菌濾器；管道的設計和安
裝應避免死角、盲管。
第一百零三條應對制藥用水及原水的水質進
行定期監測，并有相應的記錄。
第一百零四條純化水、注射用水的制備、貯
存和分配應能防止微生物的滋生，如注射用水可采
用70℃以上保溫循環。

第一百零五條應按照書面規程消毒純化水、注
射用水管道，必要時包含其它供水管道，并有相關
記錄。操作規程還應詳細規定制藥用水微生物污染
的警戒限度、糾偏限度和應采取的措施。
第六章物料與產品
第一節原則
第一百零六條應確保藥品生產所用的原輔料、
與藥品直接接觸的包裝材料符合相應的藥品注冊的
質量標準，并不得對藥品質量有不利影響。
應建立明確的物料和產品的處理和管理規程，
確保物料和產品的正確接收、貯存、發放、使用和
發運，應采取措施防止污染、交叉污染、混淆和差
錯。
第一百零七條所有物料和產品的處理，如接收、
待驗、取樣、貯存、發放及發運均應按照操作規程
或工藝規程執行，并有記錄。
第一百零八條物料和產品的運輸應能滿足質量
保證需要，對儲運條件有特殊要求的物料和產品，
其運輸條件應予確認。
第一百零九條物料應從質量管理部門批準的供
應商處采購，應對主要物料供應商進行質量審計或
評估。
第一百一十條改變物料供應商應對新的供應商
進行質量審計或評估，改變主要物料供應商還需要
對產品進行相關的驗證及穩定性考察。
第一百一十一條接收
應有原輔料、與藥品直接接觸的包裝材料和印
刷包裝材料接收的操作規程，所有到貨物料均應檢
查，以確保與訂單一致，并來自于質量管理部門批
準的供應商處。
物料的外包裝應有標簽注明規定的信息，必要
時，還應進行清潔，發現外包裝損壞或其它可能影
響物料質量的問題，應向質量管理部門報告并進行
調查和記錄。每次接收均應有記錄，記錄包括：
1.交貨單和包裝容器上所注物料的名稱；
2.企業內部所用物料名稱和（或）代碼；
3.接收日期；
4.供應商和生產商（如不同）的名稱；
5.供應商和生產商（如不同）的批號；
6.接收總量和包裝容器數量；
7.接收后企業指定的批號或流水號；
8.有關說明（如包裝狀況）。
第一百一十二條到貨物料和成品在接收或生
產后應及時按待驗管理，直至放行。
第一百一十三條物料和產品應在企業規定的
合適條件下，根據物料和產品性質有序分批貯存和
周轉，防止污染、交叉污染、混淆和差錯。
第一百一十四條物料和產品的發放及發運應
符合先進先出和近效期先出的原則。
第一百一十五條使用計算機化倉儲管理的，
應有相應的操作規程防止因系統故障、停機等特殊
情況而造成物料和產品的混淆和差錯。
第二節原輔料
第一百一十六條進口原輔料應符合藥品相關
的進口管理規定。
第一百一十七條藥品上直接印字所用油墨應
符合食用標準要求。
第一百一十八條由數個批次構成的一次收貨
的物料，應按批取樣、檢驗、放行后使用。
第一百一十九條倉儲區內的原輔料應有適當
的標識，應至少標明下述內容：
1.指定的物料名稱和企業內部的物料代碼；
2.企業接收時設定的批號；
3.物料質量狀態（如：待驗、合格、不合格、
已取樣）；
4.有效期或復驗期。
使用完全計算機化的倉儲管理系統進行識別的，

第一百二十條應有適當的操作規程或措施，
確保每一包裝內的原輔料正確無誤。
第一百二十一條只有經質量管理部門批準放
行并在有效期或復驗期內的原輔料方可使用。
第一百二十二條原輔料應按有效期或復驗期
貯存。貯存期內，如有對質量有不良影響的特殊情
況時，應進行復驗。
第一百二十三條應由指定的人員按照操作規程
進行配料，確保合格的物料經精確稱量或計量，并
作好適當標識。
第一百二十四條配制的每一物料及其重量或體
積應由他人獨立進行復核，并有復核記錄。
第一百二十五條用于同一批藥品生產的所有配
料應集中存放，并作好相應的明顯標識。
第三節中間產品和待包裝產品
第一百二十六條中間產品和待包裝產品應在適
當的條件下貯存。
第一百二十七條中間產品和待包裝產品應有明
確的標識，至少標明下述內容：
1.產品名稱和企業內部的產品代碼；
2.產品批號；
3.數量（如：毛重、凈重、皮重）；
4.生產工序（必要時）；
5.產品質量狀態（必要時，如：待驗、合格、
不合格、已取樣）
使用完全計算機化的倉儲管理系統進行識別的，
則不必以可讀的方式在標簽上標出上述信息。
第四節包裝材料
第一百二十八條與藥品直接接觸的包裝材料和
印刷包裝材料的采購、管理和控制要求與原輔料相
同。
第一百二十九條包裝材料應由專人按照操作規
程發放，應采取措施避免混淆和差錯，確保用于藥
品生產的包裝材料正確無誤。
第一百三十條應有印刷包裝材料設計、審核、
批準的操作規程。企業應確保印刷包裝材料必須與
藥品監督管理部門批準的內容、式樣、文字相一致，
并建立專門的文檔，保存經簽名批準的印刷包裝材
料原版實樣。
第一百三十一條印刷包裝材料的版本變更時，
應特別注意采取措施，確保產品印刷包裝材料的版
本正確無誤。宜收回作廢的舊版印刷模版并予以銷
毀。
第一百三十二條印刷包裝材料應存放在足夠
安全的區域內，以免未經批準人員進入。切割式標
簽或其它散裝印刷包裝材料應分別置于密閉容器內
儲運，以防混淆。
第一百三十三條印刷包裝材料應由專人保管，
并按照操作規程和需求量發放。
第一百三十四條每批或每次發放的印刷包裝
材料或與藥品直接接觸的包裝材料，均應有識別標
志，標明所用產品的名稱和批號。
第一百三十五條過期或廢棄的印刷包裝材料
或與藥品直接接觸的包裝材料，應予以銷毀并有相
應記錄。
第五節成品
第一百三十六條成品最終放行前應待驗貯存。
第一百三十七條成品的貯存條件應符合注冊
批準的要求。
第六節特殊管理的物料和產品
第一百三十八條麻醉藥品、精神藥品、醫療
用毒性藥品（包括藥材）、放射性藥品及易燃、易
爆和其它危險品的驗收、貯存、管理應執行國家有
關的規定。
第七節其它
第一百三十九條不合格的物料、中間產品、
待包裝產品和成品的每個包裝容器上均應有清晰醒
目的標志，并存放在足夠安全的隔離區內。
第一百四十條不合格的物料、中間產品、待
包裝產品和成品的任何處理均應經質量受權人的批
準并有相應記錄。
第一百四十一條只有經預先批準，方可將以
前生產的所有或部分批次合格的中間產品、待包裝
產品和成品，在某一確定的生產工序合并到同一產
品的一個批次中予以回收。應對相關的質量風險進
行充分評估后，方可按預定的操作規程進行回收處
理。回收應有相應記錄。回收批次的有效期應根據
參與回收的最早批次產品的生產日期來確定。
第一百四十二條制劑產品不得進行重新加工。
不合格的制劑中間產品、待包裝產品和成品一般不

得進行返工。只有不影響產品質量、符合相應質量
標準，且根據預定、經批準的操作規程以及對相關
風險充分評估后，才允許返工處理。返工應有相應
記錄。
第一百四十三條對返工或重新加工或回收合并
后生產的成品，質量管理部門應考慮需要進行額外
相關項目的檢驗和穩定性考察。
第一百四十四條企業應建立藥品退貨的操作規
程，并有相應的記錄，內容應至少包括：產品名稱、
批號、規格、數量、退貨單位及地址、退貨原因及
日期、最終處理意見。
同一產品同一批號不同渠道的退貨應分別記錄、
存放和處理。
第一百四十五條不符合貯存和運輸要求的退貨
產品應在質量管理部門監督下予以銷毀。只有經檢
查、檢驗和調查，有證據證明退貨產品質量未受影
響，且經質量管理部門根據操作規程嚴格評價后，
方可考慮將退貨產品重新包裝、重新發運銷售。評
價考慮的因素應至少包括藥品的性質、所需的貯存
條件、藥品的現狀、歷史，以及發運與退貨之間的
間隔時間等因素。如對藥品質量存有任何懷疑時，
退貨產品不得重新發運。
如對退貨產品進行回收處理，回收后的產品應
符合預定的質量標準和第一百四十一條的要求。任
何退貨處理均應有相應記錄。
第七章確認與驗證
第一百四十六條企業應確定需要進行的確認或
驗證工作，以證明有關操作的關鍵要素能得到有效
控制。確認或驗證的范圍和程度應經過風險評估來
確定。
第一百四十七條企業的廠房、設施、設備和檢
驗儀器應經過確認或驗證，應采用經過驗證的生產
工藝、操作規程和檢驗方法進行生產、操作和檢驗，
并保持持續的驗證狀態。
第一百四十八條應建立確認和驗證的文件和記
錄，并能以文件和記錄證明達到以下預定的目標：
1.設計確認應證明廠房、輔助設施、設備的設
計符合預定用途和本規范要求；
2.安裝確認應證明廠房、輔助設施和設備的建
造和安裝符合設計標準；
3.運行確認應證明廠房、輔助設施和設備的運
行符合設計標準；
4.性能確認應證明廠房、輔助設施和設備在正
常操作方法和工藝條件下能持續有效地符合標準要
求；
5.工藝驗證應證明一個生產工藝在規定的工藝
參數下能持續有效地生產出符合預定的用途、符合
藥品注冊批準或規定的要求和質量標準的產品。
第一百四十九條采用新的生產處方或生產工
藝前，應驗證其對常規生產的適用性。生產工藝在
使用規定的原輔料和設備條件下，應能始終生產出
符合預定的用途、符合藥品注冊批準或規定的要求
和質量標準的產品。。
第一百五十條當影響產品質量的主要因素，
如原輔料、與藥品直接接觸的包裝材料、生產設備、
生產環境（或廠房）、生產工藝、檢驗方法及其它
因素發生變更時，應進行確認或驗證，必要時，還
應經過藥品監督管理部門的批準。
第一百五十一條清潔方法應經過驗證，證實

定期進行再驗證，確保其能夠達到預期結果。
第一百五十三條企業確認和驗證工作的關鍵
信息應在驗證總計劃中以文件形式清晰說明。
第一百五十四條企業應在驗證總計劃或其它
相關文件中作出相應的規定，確保廠房、設施、設
備、檢驗儀器、生產工藝、操作規程和檢驗方法等
能保持持續的驗證狀態。
第一百五十五條應根據驗證對象制定驗證方
案，并經過審核、批準。驗證方案應明確實施驗證
的職責。
第一百五十六條驗證應按照預先確定和批準
的方案實施；驗證工作完成后，應寫出驗證報告，
并經審核、批準。驗證結果和結論（包括評價和建
議）應有記錄并存檔。
第一百五十七條應根據驗證的結果確認工藝
規程和操作規程。
第八章文件管理
第一節原則
第一百五十八條良好的文件是質量保證系統的
基本要素。本規范所指的文件包括質量標準、工藝
規程、操作規程、記錄、報告等。
應精心設計、制定、審核和發放文件，文件的
內容應與藥品生產許可、藥品注冊批準的相關要求
一致，與本規范有關的文件應經過質量管理部門的
審核。企業必須有內容正確的書面質量標準、生產
處方和工藝規程、管理和操作規程以及記錄。
文件應按照操作規程管理，內容應清晰、易懂，
并有助于追溯每批產品的歷史情況。
第一百五十九條應建立文件的起草、修訂、審
核、批準、替換或撤銷、復制、保管和銷毀等管理
制度，并有相應的文件分發、撤銷、復制、銷毀的
記錄。
第一百六十條文件的起草、修訂、審核、批準
均應由適當的人員簽名并注明日期。
第一百六十一條文件內容應確切，不能模棱兩
可；文件應標明題目、種類、目的以及文件編號和
版本號。
第一百六十二條文件應分類存放、條理分明，
便于查閱。
第一百六十三條原版文件復制時，不得產生任
何差錯；復制的文件應清晰可辨。
第一百六十四條文件應定期審核、修訂；文件
修訂后，應按規定管理，防止因疏忽造成舊版文件
的誤用。分發、使用的文件應為批準的現行文本，
已撤銷和過時的文件除留檔備查外，不得在工作現
場出現。
第一百六十五條記錄應留有數據填寫的足夠空
格。記錄應及時填寫，內容真實，字跡清晰、易讀，
不易擦掉。
第一百六十六條應盡可能采用生產和檢驗設備
自動打印的記錄、圖譜和曲線圖等，并標明產品或
樣品的名稱、批號和記錄設備的信息，操作人應簽
注姓名和日期。
第一百六十七條記錄應保持清潔，不得撕毀和
任意涂改。記錄填寫的任何
更改都應簽注姓名和日期，并使原有信息仍清晰可
辨，必要時，應說明更改的理由。記錄如需重新謄
寫，則原有記錄不得銷毀，而應作為重新謄寫記錄
的附件保存。
第一百六十八條與本規范有關的每項活動均
應有記錄，所有記錄至少應保存至藥品有效期后一
年，確認和驗證、穩定性考察的記錄和報告等重要
文件應長期保存，以保證產品生產、質量控制和質
量保證等活動可以追溯。
每批藥品應有批記錄，包括批生產記錄、批包
裝記錄、批檢驗記錄和藥品放行審核記錄等與本批
產品有關的記錄和文件。批記錄應由質量管理部門
負責管理。
第一百六十九條如使用電子數據處理系統、
照相技術或其它可靠方式記錄數據資料，應有所用
系統的詳細規程；記錄的準確性應經過核對。如果
使用電子數據處理系統，只有受權人員方可通過計
算機輸入或更改數據，更改和刪除情況應有記錄；
應使用密碼或其它方式來限制數據系統的登錄；關
鍵數據輸入后，應由他人獨立進行復核。用電子方
法保存的批記錄，應采用磁帶、縮微膠卷、紙質副
本或其它方法進行備份，以確保記錄的安全，且數
據資料在保存期內應便于查閱。
第二節質量標準
第一百七十條物料和成品應有經過批準的現
行質量標準；必要時，中間產品或待包裝產品也應
有質量標準。
第一百七十一條物料質量標準
原輔料、與藥品直接接觸的包裝材料或印刷包
裝材料的質量標準一般應包括：
1.對物料的描述，包括：
(1)企業統一指定的物料名稱和內部使用的物
料代碼；
(2)質量標準的依據；
(3)經批準的供應商；
(4)印刷包裝材料的實樣或樣稿。

第一百七十三條成品的質量標準
成品的質量標準應包括：第18頁
1.產品名稱以及產品代碼；
2.對應的產品處方編號（如有）；
3.產品規格和包裝規格；
4.取樣、檢驗方法或相關操作規程編號；
5.定性和定量的限度要求；
6.貯存條件和注意事項；
7.有效期。
第三節工藝規程
第一百七十四條每種藥品的每種生產批量均應
有相應的經企業正式批準的工藝規程，每種藥品的
每種規格和每種包裝類型均應有各自的包裝操作要
求。工藝規程的制定應以注冊批準的工藝為依據。
第一百七十五條工藝規程不得任意更改。如需
更改，應按相關的操作規程修訂、審核、批準。
第一百七十六條制劑的工藝規程的內容應包括：
1.生產處方
(1)產品名稱和產品代碼；
(2)產品劑型、規格和批量；
(3)所用原輔料清單（包括生產過程中可能消失、
不在成品中出現的物料），闡明每一物料的指定名
稱、唯一的代碼和用量；如原輔料的用量需要折算
時，還應說明計算方法；
2.生產操作要求
(1)對生產場所和所用設備的說明（如操作間的
位置和編號、潔凈度級別、必要的溫濕度要求、設
備型號和編號等）；
(2)關鍵設備的準備所采用的方法（如清洗、組
裝、校準、滅菌等）或相應操作規程編號；
(3)詳細的生產步驟說明（如物料的核對、預處
理、加入物料的順序、混合時間、溫度等）；
(4)所有中間控制方法及評判標準；
(5)預期的最終產量限度，必要時，還應說明中
間產品的產量限度，以及物料平衡的計算方法和限
度；
(6)待包裝產品的貯存要求，包括容器、標簽及
特殊貯存條件；
(7)需要說明的特別注意事項。
3.包裝操作要求
(1)以最終包裝容器中產品的數量、重量或體
積表示的包裝規格；
(2)所需全部包裝材料的完整清單，包括包裝
材料的名稱、數量、規格、類
型以及與質量標準有關的每一包裝材料的代碼；
(3)印刷包裝材料的實樣或復制品，并標明產
品批號、有效期打印位置；
(4)需要說明的特別注意事項，包括對生產區
和設備進行的檢查，在包裝操作開始前，確認包裝
生產線的清場已經完成等；
(5)包裝操作步驟的說明，包括重要的輔助性
操作條件和所用設備的注意事項、包裝材料使用前
的核對；
(6)中間控制的詳細操作，包括取樣方法及合
格標準；
(7)待包裝產品、印刷包裝材料的物料平衡計
算方法和限度。
第四節批生產記錄
第一百七十七條每批產品均應有相應的批生
產記錄，可追溯該批產品的生產歷史以及與質量有
關的情況。
第一百七十八條批生產記錄應依據現行批準
的工藝規程的相關內容制定。記錄的設計應避免抄
錄差錯。批生產記錄的每一頁應標注產品的名稱、
規格和批號。
第一百七十九條原版空白的批生產記錄應經

與包裝相關的內容制定。記錄的設計應注意避免抄
錄差錯。批包裝記錄的每一頁均應標注所包裝產品
的名稱、規格、包裝形式和批號。
第一百八十五條批包裝記錄應有待包裝產品的
批號、數量以及成品的批號和計劃數量。原版空白

效期及其它打印內容；不易隨批包裝記錄歸檔的印
刷包裝材料可采用印有上述內容的復制品；
9.對特殊問題及異常事件的注釋，包括對偏離
工藝規程的偏差情況的詳細說明或調查報告，并經
簽字批準；
10.所有印刷包裝材料和待包裝產品的名稱、
代碼，以及發放、使用、銷毀或退庫的數量、實際
產量以及物料平衡檢查。
第六節操作規程和記錄
第一百八十九條操作規程的內容應包括：題
目、編號、版本號、生效日期、分發部門以及制定
人、審核人、批準人的簽名并注明日期，標題、正
文及變更歷史。
第一百九十條廠房、設備、物料、文件和記
錄均應有編號（或代碼）的編制操作規程，應能確
保編號（或代碼）的唯一性。
第一百九十一條下述活動應有相應的操作規
程、所采取的措施或所得結果的相關記錄：
1.驗證；
2.設備的裝配和校準；
3.廠房和設備的維護、清潔和消毒；
4.培訓、著裝及衛生等與人員相關的事宜；
5.環境監測；
6.蟲害控制；

第九章生產管理
第一節原則
第一百九十二條所有藥品的生產和包裝均應按
照批準的工藝規程和操作規程進行操作并有相關記
錄，以確保藥品達到規定的質量標準，并符合藥品
生產許可和藥品注冊批準的要求。
第一百九十三條應建立產品劃分生產批次的規
定，生產批次的劃分應能確保同一批次產品質量和
特性的均一性。
第一百九十四條應建立編制藥品批號和確定生
產日期的操作規程。每批藥品均應編制唯一的批號。
除另有法定規定外，生產日期不得遲于產品成型或
灌裝（封）前經最后混合的操作日期，不得以產品
包裝日期作為生產日期。
第一百九十五條每批產品應檢查產量和物料平
衡，確保物料平衡符合設定的限度。如有差異，必
須查明原因，在得出合理解釋、確認無潛在質量風
險后，方可按正常產品處理。
第一百九十六條不同品種和規格的藥品生產操
作不得在同一生產操作間同時進行，除非沒有發生
混淆或交叉污染的可能。
第一百九十七條在生產的每一階段，應保護產
品和物料免受微生物和其它污染。
第一百九十八條在干燥物料或產品，尤其是高
活性、高毒性或高致敏性物料或產品的生產過程中，
應采取特殊措施，防止粉塵的產生和擴散。
第一百九十九條為防止混淆和差錯，生產期間
所有使用的物料、中間產品或待包裝產品的容器、
主要設備及必要的操作室應貼簽標識或以其它方式
標明生產中的產品或物料名稱、規格和批號，如有
必要，還應標明生產工序。
第二百條容器、設備或設施所用標識應清晰明
了，標識的格式應經過企業
相關部門批準。除在標識上使用文字說明外，還可
采用不同的顏色區分被標識物的狀態（如：待驗、
合格、不合格或已清潔等）。
第二百零一條應檢查產品從一個區域輸送至另
一個區域的管道和其它設備連接，確保連接正確無
誤。
第二百零二條每批藥品的每一生產階段完成
后必須由生產操作人員清場，填寫清場記錄。清場
記錄內容包括：操作間編號、產品名稱、批號、生
產工序、清場日期、檢查項目及結果、清場負責人
及復核人簽名。清場記錄應納入批生產記錄。
第二百零三條應盡可能避免出現任何偏離工
藝規程或操作規程的偏差。一旦出現偏差，應立即
報告主管人員和質量管理部門，并經簽字批準，必
要時，應由質量管理部門參與調查并作出處理。
第二百零四條生產廠房應僅限于經批準的人
員出入。
第二節防止生產過程中的污染和交叉污染
第二百零五條生產過程中應盡可能采取措施，
防止污染和交叉污染，如：
1.在分隔的區域內生產不同品種的藥品；
2.采用階段性生產方式；
3.設置必要的氣鎖間和排風；空氣潔凈度級別
不同的區域應有壓差控制；
4.應盡可能降低未經處理或未經充分處理的空
氣再次進入生產區導致污染的風險；
5.在易產生交叉污染的生產區內，操作人員應
穿戴該區域專用的防護服；
6.采用經過驗證或已知有效的清潔和去污染操
作規程進行設備清潔；必要時，應對與物料直接接
觸的設備表面的殘留物進行檢測；
7.采用密閉系統生產；
8.干燥設備的進風應有空氣過濾器，排風應有
防止空氣倒流裝置；
9.生產和清潔過程中應避免使用易碎、易脫屑、
易發霉器具；使用篩網時，應有防止因篩網斷裂而
造成污染的措施；
10.液體制劑的配制、過濾、灌封、滅菌等工
序應在規定時間內完成；
11.軟膏劑、栓劑生產中的中間產品應規定貯
存期和貯存條件。
第二百零六條應定期檢查防止污染和交叉污
染的措施并評估其適用性和有效性。

第三節生產操作
第二百零七條生產操作前，應采取措施，保證
工作區和設備已處于清潔或待用狀態，沒有任何與
本批生產無關的原輔料、遺留產品、標簽和文件。
第二百零八條生產操作前，應核對物料或中間
產品的名稱、代碼、批號和標識，確保生產所用物
料或中間產品正確且符合要求。
第二百零九條應進行中間控制和必要的環境監
測，并予以記錄。
第四節包裝操作
第二百一十條制定包裝操作規程時，應特別注
意采取措施降低污染和交叉污染、混淆或差錯的風
險。有數條包裝線同時進行包裝時，應采取隔離或
其它有效防止污染、交叉污染或混淆的措施。
第二百一十一條包裝操作前，應采取適當措施，
確保工作區、包裝生產線、印刷機及其它設備已處
于清潔或待用狀態，沒有任何與本批包裝無關的產
品、物料和文件。
第二百一十二條包裝操作前，應核對待包裝產
品和所用包裝材料的名稱、規格、數量、質量狀態，
且與工藝規程相符。
第二百一十三條每一包裝操作場所或包裝生產
線，應有標明包裝中的產品名稱、規格、批號和批
量的生產狀態標識。
第二百一十四條待分裝容器在分裝前應保持清
潔，并注意清除容器中玻璃碎屑、金屬顆粒等污染
物。
第二百一十五條通常情況下，產品分裝、封口
后應及時貼簽；否則，應按照相關的操作規程操作，
以確保不會發生混淆或貼錯標簽等差錯。
第二百一十六條任何單獨打印或包裝過程中的
打印（如產品批號或有效期）均應進行檢查，確保
其正確無誤，并予以記錄。應特別注意手工打印內
容并定期復核。
第二百一十七條使用切割式標簽，以及在包裝
線以外打印標簽時，應有專門的管理措施，防止混
淆。
第二百一十八條應對電子讀碼機、標簽計數器
或其它類似裝置的功能進行檢查，確保其準確運行。
檢查應有記錄。
第二百一十九條包裝材料上印刷或模壓的內
容應清晰、不褪色、不易擦去。
第二百二十條包裝期間，產品的在線控制檢
查至少應包括下述各項內容：
1.包裝外觀；
2.包裝是否完整；
3.產品和包裝材料是否正確；
4.打印內容是否正確；
5.在線監控裝置的功能是否正常。
樣品從包裝生產線取走后不應再返還，以防止
產品混淆或污染。
第二百二十一條只有經過專門檢查、調查，
并由指定人員批準后，出現異常情況時的產品方可
重新包裝。此過程應有詳細記錄。
第二百二十二條在物料平衡檢查中，發現待
包裝產品、印刷包裝材料以及成品數量有顯著差異
或異常時，應進行調查，未得到合理解釋前，成品
不得放行。
第二百二十三條包裝結束時，已打印批號的
剩余包裝材料應由專人負責全部計數銷毀，并有記
錄。如將未打印批號的印刷包裝材料退庫，應嚴格
按照操作規程執行。
第十章質量控制與質量保證
第一節質量控制實驗室管理
第二百二十四條質量控制實驗室的人員、場
所、設備應同生產操作的性質和規模相適應。如有
特殊原因，可以按照第十一章中委托檢驗部分闡述
的原則，委托外部實驗室進行檢驗，但應在檢驗報
告中予以說明。
第二百二十五條質量控制負責人應具有足夠
的管理實驗室的資質和經驗，可以管轄同一企業的
一個或多個實驗室。
第二百二十六條質量控制實驗室的檢驗人員
應至少具有相關專業中專或高中以上的學歷，并至
少經過一定時限與所從事的檢驗操作相關的實踐培
訓且通過考核。
第二百二十七條質量控制實驗室應配備藥典、
標準圖譜等必要的工具書，還應具有標準品或對照
品等相關的標準物質。
第二百二十八條文件
1.質量控制實驗室文件應符合第八章的原則，
應有下列詳細文件，如：
(1)質量標準；
(2)取樣操作規程和記錄；
(3)檢驗操作規程和記錄（包括檢驗記錄或實驗
室工作記事簿）；
(4)檢驗報告或證書；
(5)必要的環境監測操作規程、記錄和報告；
(6)必要的檢驗方法驗證記錄；
(7)儀器校準和設備維護的操作規程及記錄。
2.每批藥品的檢驗記錄應包括中間產品、待包
裝產品和成品的質量檢驗記錄，可追溯該批藥品所
有相關的質量檢驗情況。
3.宜采用便于趨勢分析的方法保存某些數據
（如檢驗數據、環境監測數據、制藥用水的微生物
監測數據）。
4.除與批記錄相關的資料信息外，還應保存其
它原始資料，如實驗室日志或記錄，以方便查閱。
第二百二十九條取樣
應有取樣的操作規程，包括：經授權的取樣人、
取樣方法、取樣用設備、取樣量、取樣后剩余部分
及樣品的處置和標識，以及為避免因取樣過程產生
的各種風險的預防措施等。
1.質量管理部門的人員有權進入生產區和倉儲
區進行取樣及調查。
2.應按照經批準的操作規程取樣，操作規程應
詳細闡述：
(1)取樣方法；
(2)所用器具；
(3)樣品量；
(4)分樣的方法；
(5)存放樣品容器的類型和狀態；
(6)樣品容器的標識；
(7)取樣注意事項，尤其是無菌或有害物料的取
樣以及防止取樣過程中污染和交叉污染的注意事項；
(8)貯存條件；
(9)取樣器具的清潔方法和貯存要求。
3.取樣方法應科學、合理，以保證樣品的代表
性。
4.留樣應能代表被取樣批次的產品或物料，也
可抽取其它樣品來監控生產過程中最重要的環節
（如生產的開始或結束）。
5.樣品的容器應貼有唯一性的標簽標明內容物，
注明樣品名稱、批號、取樣日期、取自哪一包裝容
器、取樣人。
6.樣品應按規定的貯存條件保存，成品的樣品
應按照注冊批準的成品貯存條件保存。
第二百三十條檢驗
應有物料和不同生產階段產品的書面檢驗操作
規程，闡述所用方法、儀器和設備。檢驗結果應有
記錄。
1.企業應確保藥品按照注冊批準的方法進行全
項檢驗。
2.符合下列情形之一的，應對檢驗方法進行驗
證：
(1)采用新的檢驗方法；
(2)檢驗方法需變更的；
(3)采用《中華人民共和國藥典》及其它法定
標準未收載的檢驗方法；
(4)法規規定的其它需要驗證的檢驗方法。
3.對不需要進行驗證的檢驗方法，企業應對檢
驗方法進行確認，以確保檢驗數據準確、可靠。
4.檢驗應有可追溯的記錄并應復核，確保結果
與記錄一致。所有計算均應嚴格核對。
5.檢驗記錄至少應包括以下內容：
(1)產品或物料的名稱、劑型、規格、批號或
供貨批號，必要時注明供應商和生產商（如不同）
的名稱或來源；
(2)依據的質量標準和檢驗操作規程；
(3)檢驗所用的儀器或設備的型號和編號；
(4)檢驗所用的試液和培養基的配制批號、對
照品或標準品的來源和批號；
(5)檢驗所用動物的相關信息；
(6)檢驗過程，包括對照品溶液的配制、各項
具體的檢驗操作、必要的環境溫濕度；
(7)檢驗結果，包括觀察情況、計算和圖譜或
曲線圖，以及依據的檢驗報告編號；

6.所有中間控制（包括生產人員所進行的中間
控制），均應按經質量管理部門批準的方法進行，
檢驗應有記錄。
7.應特別注意實驗室容量分析用玻璃儀器、試
劑、試液、對照品以及培養基的質量。
8.原輔料、與藥品直接接觸的包裝材料或產品
檢驗用動物，必要時應在使用前檢驗或隔離檢疫。
飼養和管理應確保動物適用于預定用途。動物應有
標識，并應保存使用的歷史記錄。
第二百三十一條檢驗結果超標調查
質量控制實驗室應建立檢驗結果超標調查的操
作規程。任何檢驗結果超標都必須按照操作規程進
行完整的調查，并有相應的記錄。
第二百三十二條留樣
1.企業按規定保存的、用于藥品質量追溯或調
查的物料、產品樣品為留樣。用于產品穩定性考察
的樣品不屬于留樣。
2.應按照經批準的操作規程對留樣進行管理。
3.留樣應能代表被取樣批次的物料或產品。
4.成品的留樣
(1)每批藥品均應有留樣；如果一批藥品分成數
次進行包裝，則每次包裝應至少保留一件最小市售
包裝的成品。
(2)留樣的包裝形式應與藥品市售包裝形式相同，
原料藥的留樣如不采用市售包裝形式的，可采用模
擬包裝。
(3)每批藥品的留樣數量一般應至少能確保按照
注冊批準的質量標準完成二次全檢（無菌檢查和熱
原檢查等除外）。
(4)如果不影響留樣的包裝完整性，保存期間內
應至少每年對留樣進行一次目檢觀察，如有異常，
應進行徹底調查并采取相應的處理措施。
(5)留樣觀察應有記錄。
(6)留樣應按注冊批準的貯存條件至少保存至藥
品有效期后一年。
(7)如企業終止藥品生產或關閉的，應將留樣轉
交受權單位保存，并告知當地藥品監督管理部門，
以便在必要時可隨時取得留樣。
5.物料的留樣
(1)制劑生產用每批原輔料和與藥品直接接觸
的包裝材料均應有留樣，與藥品直接接觸的包裝材
料（如輸液瓶），如成品已有留樣，可不必單獨留
樣。
(2)物料的留樣量應至少滿足鑒別要求。
(3)除穩定性較差的原輔料外，用于制劑生產
的原輔料（不包括生產過程中使用的溶劑、氣體或
制藥用水）和與藥品直接接觸的包裝材料的留樣應
至少保存至產品放行后二年。如果物料的有效期較
短，則留樣時間可相應縮短。
物料的留樣應按規定的條件貯存，必要時還應
適當包裝密封。
第二百三十三條試劑、試液、培養基和檢定
菌
1.試劑和培養基應從可靠的供應商處采購，必
要時應對供應商進行評估。
2.應有接收試劑、試液、培養基的記錄，必要
時，應在試劑、試液、培養基的容器上標注接收日
期。
3.應按照相關規定或使用說明配制、貯存和使
用試劑、試液和培養基。特殊情況下，在接收或使
用前，還應對試劑進行鑒別或其它檢驗。
4.試液和已配制的培養基應標注配制批號、配
制日期和配制人員姓名，并有配制（包括滅菌）記
錄。不穩定的試劑、試液和培養基應標注有效期及
特殊貯存條件。標準液、滴定液還應標注最后一次
標化的日期和校正因子，并有標化記錄。
5.每次配制的培養基均應進行無菌性和靈敏度
檢查，并有相關記錄。應有培養基使用的記錄。
6.應有檢驗所需的各種檢定菌，并建立檢定菌
保存、傳代、使用、銷毀的操作規程和相應記錄。
7.檢定菌應有適當的標識，內容至少包括菌種
名稱、編號、代次、傳代日期、傳代操作人。
8.檢定菌應按規定的條件貯存，貯存的方式和
時間不應對檢定菌的生長特性有不利影響。
第二百三十四條標準品或對照品
1.標準品或對照品應按規定貯存和使用。
2.標準品或對照品應有適當的標識，內容至少
包括名稱、批號、制備日期、有效期（如有）、首
次開啟日期、含量或效價、貯存條件。
3.企業自制工作標準品或對照品的，應建立工
作標準品或對照品的質量標準以及制備、鑒別、檢
驗、批準和貯存的操作規程，每批工作標準品或對
照品應用法定標準品或對照品進行標化，并確定有
效期，還應通過定期標化證明工作標準品或對照品
的效價或含量在有效期內保持穩定。標化應有相應
的記錄。
第二節物料和產品放行
第二百三十五條應分別建立物料和產品批準放
行的操作規程，明確批準放行的標準、職責，并有
相應的記錄。
第二百三十六條物料的放行
1.物料的質量評價內容應至少包括生產商的檢
驗報告、物料包裝完整性、
密封性的檢查情況和檢驗結果。
2.物料的質量評價應有明確的結論，如批準放
行、不合格或其它決定。
3.物料的放行應由指定人員簽名批準放行。
第二百三十七條產品的放行
1.在批準放行前，應對每批藥品進行質量評價，
保證藥品及其生產應符合注冊批準或規定的要求和
質量標準，主要生產工藝和檢驗方法經過驗證，保
證藥品的生產符合本規范要求，并確認符合以下各
項要求：
(1)已完成所有必需的檢查、檢驗，并綜合考慮
實際生產條件和生產記錄；
(2)所有必須的生產和質量控制均已完成并由經
相關主管人員簽名；
(3)變更已按照相關規程處理完畢，需要經藥品
監督管理部門批準的變更已得到批準；
(4)對變更或偏差已完成所有額外的取樣、檢查、
檢驗和審核；
(5)所有與該批產品有關的偏差均已有明確的解
釋或說明，或者已經過徹底調查和適當處理；如偏
差還涉及其它批次產品，應一并處理。
2.藥品的質量評價應有明確的結論，如批準放
行、不合格或其它決定。
3.每批經批準放行的藥品均應由質量受權人簽
名放行。
4.生物制品和血液制品的放行還應符合《生物
制品批簽發管理辦法》的要求。
第三節持續穩定性考察
第二百三十八條應進行適當的持續穩定性考
察，監控已上市藥品的穩定性，以發現市售包裝藥
品與生產相關的任何穩定性問題（如雜質含量或溶
出度特性的變化）。
第二百三十九條持續穩定性考察的目的是在
有效期內監控藥品質量，并確定藥品可以或預期可
以在標示的貯存條件下，符合質量標準的各項要求。
第二百四十條持續穩定性考察主要針對市售
包裝藥品，但也需兼顧待包裝產品。例如，當待包
裝產品在完成包裝前，或從生產廠運輸到包裝廠，
還需要長期貯存時，應在相應的環境條件下，評估
其對包裝后產品穩定性的影響。此外，還應考慮對
貯存時間較長的中間產品進行考察。
第二百四十一條持續穩定性考察應有考察方
案，結果應有報告。用于持續穩定性考察的設備
（尤其是穩定性試驗設備或設施）應按照第七章和
第五章的要求進行確認和維護。
第二百四十二條考察方案應涵蓋藥品有效期，
應至少包括以下內容：
1.每種規格、每種生產批量藥品的考察批次；
2.相關的物理、化學、微生物和生物學檢驗方
法，可考慮采用穩定性考察專屬的檢驗方法；
3.檢驗方法依據；
4.合格標準；
5.容器密封系統的描述；
6.試驗間隔時間（測試時間點）；
7.貯存條件（應采用與藥品標示貯存條件相對
應的《中華人民共和國藥典》長期穩定性試驗的標
準條件）；
8.檢驗項目，如檢驗項目少于成品質量標準所
包含的項目，應說明理由。
第二百四十三條考察批次數和檢驗頻率應能
獲得足夠的數據，以供趨勢分析。通常情況下，每

一個批次，除非當年沒有生產。
第二百四十四條某些情況下，持續穩定性考察
中應額外增加批次，如重大變更或生產和包裝有重
大偏差的藥品應列入穩定性考察。此外，任何采用
非常規工藝重新加工、返工、或有回收操作的批次，
也應考慮列入考察，除非已經過驗證和穩定性考察。
第二百四十五條關鍵人員，尤其是質量受權人，
應了解持續穩定性考察的結果。當持續穩定性考察
不在待包裝產品和成品的生產企業進行時，則相關
各方之間應有書面協議。生產企業應保存持續穩定
性考察的結果以供藥品監督管理部門審查。
第二百四十六條應對不符合質量標準的結果或
重要的異常趨勢進行調查。任何已確認的不符合質
量標準的結果或重大不良趨勢，都應向當地藥品監
督管理部門報告；企業還應考慮是否可能對已上市
藥品造成影響，必要時應實施召回。
第二百四十七條應根據所獲得的全部數據資料，
包括考察的階段性結論，撰寫總結報告并保存。應
定期審核總結報告。
第四節變更控制
第二百四十八條企業應建立變更控制系統，對
所有影響產品質量的變更進行評估和管理。需要經
藥品監督管理部門批準的變更應在得到批準后方可
實施。
第二百四十九條應建立操作規程規定原輔料、
包裝材料、質量標準、檢驗方法、操作規程、廠房、
設施、設備、儀器、生產工藝和計算機軟件變更的
申請、評估、審核、批準和實施。質量管理部門應
指定專人負責變更控制。
第二百五十條任何申請的變更都應評估其對產
品質量的潛在影響。企業可以根據變更的性質、范
圍、對產品質量潛在影響的程度將變更分類（如主
要、次要變更）。判斷變更所需的驗證、額外的檢
驗以及穩定性考察應有科學依據。
第二百五十一條任何與本規范有關的變更經申
請部門提出后，應由質量管理部門評估、審核和批
準，制定變更實施的計劃，明確實施的職責分工，
并監督實施。變更實施應有相應的完整記錄。
第二百五十二條改變原輔料、與藥品直接接觸
的包裝材料、生產工藝、主要生產設備以及其它影
響藥品質量的主要因素時，還應對變更實施后最初
至少三個批次的藥品質量進行評估。如果變更可能
影響藥品的有效期，則質量評估還應包括對變更實
施后生產的藥品進行穩定性考察。
第二百五十三條變更實施時，應確保與變更
相關的文件均已修訂。
第二百五十四條質量管理部門應保存所有變
更的文件和記錄。
第五節偏差處理
第二百五十五條各部門負責人應確保所有人
員嚴格、正確執行預定的生產工藝、質量標準、檢
驗方法和操作規程，防止偏差的產生。
第二百五十六條企業應建立偏差處理的操作
規程，規定偏差的報告、記錄、調查、處理程序或
所采取的糾正措施，并有相應的記錄。
第二百五十七條任何偏差都應評估其對產品
質量的潛在影響。企業可以根據偏差的性質、范圍、
對產品質量潛在影響的程度將偏差分類（如重大、
次要偏差），對重大偏差的評估還應考慮是否需要
對產品進行額外的檢驗以及對產品有效期的影響，
必要時，應對涉及重大偏差的產品進行穩定性考察。
第二百五十八條任何偏離預定的生產工藝、
物料平衡限度、質量標準、檢驗方法、操作規程等
的情況均應有記錄并立即報告主管人員及質量管理
部門，應有清楚的解釋或說明，重大偏差應由會同
其它部門進行徹底調查，并有調查報告。偏差調查
報告應由質量管理部門的指定人員審核并簽字。
企業還應采取充分的預防措施有效防止類似偏
差的再次發生。
第二百五十九條質量管理部門應負責偏差的
分類，保存偏差調查、處理的文件和記錄。
第六節糾正和預防措施
第二百六十條企業應建立糾正和預防措施系
統，對投訴、召回、偏差、自檢或外部檢查結果、
工藝性能和質量監測趨勢等進行調查并采取糾正和
預防措施。調查的深度和形式應與風險的級別相適
應。糾正和預防措施系統采用的方法應能改進產品
和工藝，增進對產品和工藝的理解。
第二百六十一條企業應建立實施糾正和預防措
施的操作規程，內容包括：
1.對投訴、召回、偏差、自檢或外部檢查結果、
工藝性能和質量監測趨勢以及其它來源的質量數據
進行分析，確定已有和潛在的質量問題。必要時，
應采用適當的統計學方法；
2.調查與產品、工藝和質量保證系統有關的原
因；
3.確定所需采取的糾正和預防措施，防止問題
的再次發生；
4.評估糾正和預防措施的合理性、有效性和充
分性；
5.對實施糾正和預防措施過程中所有發生的變
更應予以記錄；
6.確保相關信息已傳遞到質量受權人和預防問
題再次發生的直接負責人；
7.確保相關信息及其糾正和預防措施已通過高
層管理人員的評審。
第二百六十二條實施糾正和預防措施應有文件
記錄，并由質量管理部門保存。
第七節供應商的審計和批準
第二百六十三條質量管理部門應對所有生產用
物料的供應商進行質量評估，會同有關部門對主要
物料供應商（尤其是生產商）的質量體系進行現場
質量審計，并對質量審計或評估不符合要求的供應
商行使否決權。
主要物料的確定應綜合考慮企業所生產的藥品
質量風險、物料用量以及物料對藥品質量的影響程
度等因素。
企業法定代表人、企業負責人及其它部門的人
員不得干擾或妨礙質量管理部門對物料供應商獨立
作出質量評估。
第二百六十四條應建立物料供應商審計和批準
的操作規程，明確供應商的資質、選擇的原則、質
量評估方式、質量審計內容、評估標準、質量審計
人員的組成及資質，確定現場質量審計周期以及物
料供應商批準的程序。
第二百六十五條質量管理部門應指定專人負責
物料供應商質量審計和質量評估，分發經批準的合
格供應商名單。被指定的人員應具有相關的法規和
專業知識，具有足夠的質量審計和評估的實踐經驗。
第二百六十六條現場質量審計應核實供應商
資質證明文件和檢驗報告的真實性，核實是否具備
檢驗條件。應對其人員機構、廠房設施和設備、物
料管理、生產工藝流程和生產管理、質量控制實驗
室的設備、儀器、文件管理等進行檢查，以全面評
估其質量保證體系。現場質量審計應有報告。
第二百六十七條必要時，應對主要物料供應
商提供的樣品進行小批量試生產，并對試生產的藥
品進行穩定性考察。
第二百六十八條質量管理部門對物料供應商
的評估應至少包括：供應商的資質證明文件、質量
標準、檢驗報告、企業對物料樣品的檢驗數據和報
告。如進行現場質量審計和樣品小批量試生產的，
還應包括現場質量審計報告，以及小試產品的質量
檢驗報告和穩定性考察報告。
第二百六十九條質量管理部門應向物料管理
部門分發經批準的合格供應商名單，該名單內容至
少包括物料名稱、規格、質量標準、生產商名稱和
地址、經銷商名稱（如有）等，并及時更新。
第二百七十條質量管理部門應定期對物料供
應商進行評估或現場質量審計，回顧分析物料質量
檢驗結果、質量投訴和不合格處理記錄。如物料出
現質量問題或生產條件、工藝、質量標準和檢驗方
法等可能影響質量的關鍵因素發生重大改變時，還
應盡快進行相關的現場質量審計。
第二百七十一條質量管理部門應與主要物料
供應商簽訂質量協議，在協議中應明確雙方所承擔
的質量責任。
第二百七十二條企業應對每家物料供應商建
立質量檔案，檔案內容應包括供應商的資質證明文
件、質量協議、質量標準、樣品檢驗數據和報告、
供應商的檢驗報告、現場質量審計報告、產品穩定
性考察報告、定期的質量回顧分析報告等。
第八節產品質量回顧分析
第二百七十三條應每年對所有生產的藥品按
品種進行產品質量回顧分析，
以確認工藝穩定可靠，以及原輔料、成品現行
質量標準的適用性，及時發現不良趨勢，確定產品
及工藝改進的方向。應考慮以往回顧分析的歷史數
據，回顧分析應有文件記錄。

以確保內容更新。
第二百七十四條應當對回顧分析的結果進行評
估，并有是否需要采取糾正和預防措施或進行再驗
證的評估意見。應有文件和記錄說明采取糾正和預
防措施的理由。應及時、有效地完成經批準的整改
計劃。應有檢查、回顧這類措施的管理規程，自檢
過程中，應檢查管理規程的有效性。當有合理的科
學依據時，可按產品的劑型分類進行質量回顧，如
固體制劑、液體制劑和無菌制劑等。
第二百七十五條當企業為藥品委托生產的受托
方時，委托方和受托方之間應有書面的技術協議，
規定產品質量回顧分析中各方的責任，負責最終產
品放行的受權人應確保質量回顧分析按時進行并符
合要求。
第九節投訴
第二百七十六條應根據操作規程，詳細審核所
有的投訴以及有關產品潛在質量缺陷的其它信息。
第二百七十七條所有產品缺陷的投訴，都應詳
細記錄投訴的各個細節，并徹底進行調查。
第二百七十八條應有專人及足夠的輔助人員
負責進行質量投訴的調查和處理，如投訴處理負責
人不是質量受權人，則所有投訴、調查的信息應向
質量受權人通報。
第二百七十九條應有操作規程，詳細闡述因
可能的產品缺陷發生投訴時應采取的措施，包括考
慮是否有必要從市場召回藥品。
第二百八十條如果發現或懷疑某批藥品存在
缺陷，應考慮檢查其它批次的藥品，以查明其是否
受到影響。
第二百八十一條因投訴作出的所有決定和采
取的措施，均應有記錄，并應注明所查相關批記錄
的信息。
第二百八十二條應定期檢查投訴記錄，以便
及時發現需引起注意的問題，以及可能需要從市場
召回藥品的特殊問題或重復出現的問題。
第二百八十三條如企業出現生產失誤、藥品
變質或其它重大質量問題，在考慮采取相應措施的
同時，還應及時向當地藥品監督管理部門報告。
第十一章委托生產與委托檢驗
第一節原則
第二百八十四條為避免因誤解而影響產品或
工作質量，委托生產或檢驗必須正確界定、經雙方
同意并嚴格控制。委托方和受托方必須簽訂書面合
同，明確規定各方的職責。合同必須明確說明質量
受權人在批準放行銷售每一批產品時，如何履行其
全部職責。
第二百八十五條應有書面合同，闡明委托生
產或委托檢驗的內容及相關的技術事項。
第二百八十六條委托生產與委托檢驗的所有
活動，包括在技術或其它方面擬采取的任何變更，
均應符合有關藥品注冊批準的要求。
第二節委托方
第二百八十七條委托方應負責對受托方進行
評估，對受托方的條件、技術水平、質量管理情況
進行詳細考查，確認其順利完成委托工作的能力，
確認采用委托方式仍能保證遵照執行本規范闡述的
原則和要求。
第二百八十八條委托方應向受托方提供所有必
要的資料，以使受托方能夠按藥品注冊批準和其它
法定要求正確實施所委托的操作。委托方應讓受托
方充分了解與產品或操作相關的各種問題，包括產
品或操作有可能對受托方的廠房、設備、人員及其
它物料或產品造成的危害。
第二百八十九條委托方應對受托生產或檢驗的
全過程進行指導和監督。
第二百九十條委托方應確保受托方發運來的所
有已生產的產品和物料均符合相應的質量標準，或
該產品已由質量受權人批準放行。
第三節受托方
第二百九十一條受托方必須具備足夠的廠房、
設備、知識和經驗以及稱職人員，以順利完成委托
方所委托的生產或檢驗工作。
第二百九十二條受托方應確保所有收到的物料、
中間產品和待包裝產品適用于預定用途。
第二百九十三條受托方不得從事任何可能對委
托生產或檢驗的產品質量有不利影響的活動。

第四節合同
第二百九十四條委托方與受托方之間應簽訂合
同，詳細規定各自的產品生產和控制職責，其中的
技術性條款應由具有制藥技術、檢驗專業知識和熟
悉本規范的主管人員擬訂。委托生產及檢驗的各項
工作必須符合藥品注冊批準的要求并經雙方同意。
第二百九十五條合同應詳細規定質量受權人批
準放行銷售每批藥品的方式，確保每批產品都已按
藥品注冊批準的要求完成生產和檢驗。
第二百九十六條合同應闡明何方負責物料的采
購、檢驗、批準放行使用、生產和質量控制（包括
中間控制）；同時也應明確何方負責取樣和檢驗。
在委托檢驗的情況下，合同應闡明受托方是否在委
托方的廠房內取樣。
第二百九十七條生產、檢驗和發運記錄及樣品
應由委托方保存；或由受托方保存，但委托方能夠
隨時調閱或檢查。在委托方質量缺陷處理或藥品召
回處理的操作規程中應有明確而詳細的規定，即當
出現投訴或懷疑有缺陷時，委托方必須能夠方便地
查閱所有與評價產品質量相關的記錄。
第二百九十八條合同應允許委托方對受托方進
行檢查或質量審計。
第二百九十九條委托檢驗合同應明確受托方同
意接受藥品監督管理部門檢查。
第十二章產品發運與召回
第一節原則
第三百條企業應建立產品召回系統，必要時可
迅速、有效地從市場召回任何一批有質量缺陷或懷
疑有質量缺陷的所有產品。
第三百零一條除有證據證明退貨產品質量未受
影響外，因質量原因退貨和召回的產品，均應按規
定監督銷毀。
因質量原因退貨和召回的產品，如涉及其它批
次或產品，應同樣處理。
第二節發運
第三百零二條每批產品均應有發運記錄。根據
發運記錄，應能追查每批產品的售出情況，必要時
應能及時全部追回，發運記錄內容應包括：產品名
稱、批號、規格、數量、收貨單位和地址、聯系方
式、發貨日期。
第三百零三條藥品發運的零頭包裝只限兩個
批號為一個合箱，合箱外應標明全部批號，并建立
合箱記錄。
第三百零四條發運記錄應保存至藥品有效期
后一年。
第三節召回
第三百零五條應指定專人負責執行召回及協
調相關工作，并應根據情況的緊急程度配備足夠數
量的人員參與這項工作。產品召回負責人一般應獨
立于銷售和市場部門；如召回負責人不是質量受權
人，則應向質量受權人通報所有產品召回處理情況。
第三百零六條為良好組織所有召回工作，應
制定書面的召回處理操作規程，并定期檢查，必要
時進行修訂。
第三百零七條召回處理應能隨時啟動，并迅
速實施。
第三百零八條如因產品有不良事件或有缺陷
或懷疑有缺陷而擬從市場召回時，應立即向當地藥
品監督管理部門報告。
第三百零九條負責產品召回的人員應能迅速
查閱到藥品發運記錄。
第三百一十條已召回的產品應有標識，并單
獨貯存在安全的區域內，等待最終處理決定。
第三百一十一條召回的進展過程應有記錄，
并有最終報告，內容包括產品發運數和召回數之間
的數量平衡。
第三百一十二條應定期對產品召回系統的有
效性進行評估。
第十三章自檢
第一節原則
第三百一十三條質量管理部門應定期組織對
企業進行自檢，以監控本規范的實施情況，評估企
業是否符合本規范要求，并提出必要的糾正和預防
措施。
第二節自檢
第三百一十四條自檢應有計劃，對機構與人
員、廠房與設施、設備、物料與產品、確認與驗證、
文件管理、生產管理、質量控制與質量保證、委托
生產與委托檢驗、產品發運與召回等項目定期進行
檢查，以確認其符合本規范的要求。
第三百一十五條應由企業指定人員獨立、細致
地進行自檢，也可請外部人員或專家進行獨立的質
量審計。
第三百一十六條自檢應有記錄。自檢完成后應
有自檢報告，內容應包括自檢過程中觀察到的所有
情況、評價的結論以及提出糾正和預防措施的建議。
此外，糾正和預防措施的執行也應有相應的記錄。
第十四章術語
本規范下列術語（按漢語拼音排序）的含義是：
1.包裝
待包裝產品變成成品所需的所有操作步驟，包
括分裝、貼簽等。但無菌生產工藝中產品的無菌灌
裝，以及最終滅菌產品的灌裝通常均不視為包裝。
2.包裝材料
藥品包裝所用的任何材料，包括與藥品直接接
觸的包裝材料和印刷包裝材料，但不包括發運用的
外包裝材料。
3.操作規程
經批準用來指導藥品生產活動的通用性文件，
如設備操作、維護與清潔、驗證、環境控制、取樣
和檢驗等，也稱標準操作規程。
4.成品
已完成生產所有操作步驟和最終包裝的產品。
5.產品
指藥品的中間產品、待包裝產品或成品。
6.產品生命周期
產品從最初的研發、上市直至退市的所有階段。
7.重新加工
將某一生產工序生產的不符合質量標準的一批
中間產品或待包裝產品的一部分或全部，采用不同
于常規生產工藝進行再加工，以符合預定的質量標

8.待包裝產品
尚未進行包裝但已完成所有其它加工工序的任

9.待驗

別的房間之間）的具有兩扇或多扇門的隔離空間。
